Our office works with a full spectrum of specialists across your South Philadelphia community, including, pharmacists, nutritionists, behavioral health specialists, and ancillary service and support personnel who work collaboratively to offer you a continuum of care that meets all of your health care needs.

Dr. Tara Cuda serves patients of all ages and diverse backgrounds. In fact, she is a hospice medical director for Holisticare Hospice to better serve the patients by providing a continuum of care.

Providing affordable, quality care is essential to remain vibrant, strong and healthy. Continuing to deliver this level of care requires that, collectively, we the reduce healthcare spend while increasing the value we place on prevention and management of chronic conditions. Reducing trips to the Emergency Room, avoiding preventable inpatient hospital stays, and implementing telehealth when appropriate are all ways we're doing this today. Providing the best patient experience I can is important to me, so in 2016, I became a recognized Patient Center Medical Home, which means that I work with my patients in a coordinated, team-based approach with all of their physicians; in other words, my patients' care is not provided in isolation, but rather with their family, their specialists, their health insurer and many others.


A cornerstone of the Patient Centered Medical Home is that care is delivered based on keeping you healthy, vs. delivering care that on a fee-for-service basis. This means that, as physicians, we operate in a value-based setting where if you remain healthy, everybody wins - including you, the health insurers and/or government as well as practicing physicians.


The concepts behind the Patient Centered Medical Home model was developed in 1967 by the American Academy of Pediatrics, but it wasn't until the early 2000's  when the existing Primary Care was struggling to survive that it was revisited and develop into an operational, measurable idea.  Find out more information on PCMH or NCQA. 

About the Doctor

Born in Pittsburgh, college in Colorado where my love for the outdoors flourished, and medical school in Philadelphia where I live and have practiced medicine for the past 23 years. My passion is cycling and skiing and I am always trying to educate my patients on the value of strengthening their bodies to maintain a healthy independent life.
